Metformin and GLP-1s lower glucose in different ways, which is exactly why they pair well. Metformin works mainly by reducing the amount of glucose your liver produces and improving how your body uses insulin, with an effect spread across the day. A GLP-1 works mostly on your meals, flattening the post-meal spikes by slowing digestion and timing insulin to the rise. One targets the background, the other targets the peaks. Combined, they cover more of the glucose picture than either alone.

Why it's a low-hypoglycemia pairing

Here's the reassuring part. Neither metformin nor a GLP-1 tends to cause low blood sugar on its own, because neither forces insulin out regardless of your glucose level. Much of the trial evidence for GLP-1 benefit comes from adding the drug on top of metformin, where the combination improves glucose control with minimal time in hypoglycemia. That's a meaningfully safer profile than adding insulin or a sulfonylurea, which do carry real hypoglycemia risk. This applies to the type 2 diabetes setting where the combination is studied.

What a CGM typically shows

On a sensor, the combination usually reads as steadier days: flatter post-meal curves from the GLP-1, a lower overall baseline helped by metformin, and time below range staying low because neither drug drives you into lows. If you started metformin first and then added a GLP-1, you'll often see the post-meal peaks specifically get smaller once the GLP-1 comes on board. "Do I still need metformin?"

A common question is whether the GLP-1 makes metformin redundant. That's a decision for your clinician, not something to change on your own. Metformin is inexpensive, well established, and has effects beyond glucose that your clinician may want to keep.
