Compounded semaglutide and tirzepatide filled a gap during the shortages, but the FDA declared those shortages resolved (tirzepatide December 2024, semaglutide February 2025), and mass compounding of copies is now largely restricted. Compounded products can also vary in concentration, which is one more reason to watch your own glucose response.

During the official shortages, pharmacies were permitted to compound semaglutide and tirzepatide. That window has closed. The FDA determined the tirzepatide shortage resolved in December 2024 and the semaglutide shortage in February 2025, with grace periods for compounders that ran through the first half of 2025 (FDA 2025). Now that the shortages are over, compounders generally cannot make products that are "essentially a copy" of the approved drugs unless a prescriber documents a significant difference needed for a specific patient. This area is legally sensitive and still evolving, so confirm the current status with your prescriber and pharmacy rather than relying on any single article, including this one.
Why compounded products can behave differently on your glucose
Brand-name pens deliver a standardized, precisely measured dose. Compounded products are prepared by individual pharmacies and can differ in concentration, formulation, and sometimes the exact form of the active ingredient.
